Fax Scheduling

Send us your orders by fax to 901-680-1902 and we will contact the patient to schedule their appointment. Please include the following with your fax:

• Completed and signed order form including patient contact info
• Copy of patient’s insurance card – both front and back
• If pre-certification assistance is needed, send pertinent clinical notes for MRI, MRA, CT, CTA & Nuclear Imaging studies only

Pre-certification Assistance

We are pleased to provide assistance in obtaining the patient’s insurance pre-certification for their procedure if required. If assistance is needed, we must have pertinent clinical notes from the referring provider to obtain the pre-cert.

Most insurance carriers require pre-certification for MRI, MRA, CT, and CTA studies although some carriers require pre-certification for Ultrasound exams as well. You may fax the information with your order and we will handle scheduling the patient and obtaining the pre-cert.
